Duties and Responsibilities
The Field RN (Registered Nurse) performs home visits, assessments and follow up visits to patients.
The RN advocates for quality patient care, collaborates with Physicians and other providers to maintain a safe and effective home environment for patients.
The Registered Nurse works closely with patients, families and interdisciplinary team to ensure quality care is provided at home.
Job Qualifications Required
Must be a NYS Registered Nurse
Experience working for a CHHA or Prior Hospital Med/Surg experience
Ability to visit patients in a home environment
Willingness to collaborate with interdisciplinary team as needed
Excellent organizational skills required
Education, Knowledge, Skills and Abilities Preferred
Knowledge of ICD 10, Medicare/Medicaid and OASIS guidelines a plus
Ability to drive and have access to reliable automobile preferred
Bilingual Chinese, (Cantonese/Mandarin) Russian, or Spanish a plus

The check will bounce and the victim is left responsible.
